Genesis Health Inc. has filed plans for Brooks University Crossing, a skilled nursing center designed at Beach and University boulevards.

The St. Johns River Water Management District is reviewing an application for the three-story, 86,000-square-foot center planned on 8.34 acres.

As the Daily Record reported May 3, Brooks Rehabilitation intends to build the skilled-nursing facility at the site of the former Gator Office Furniture building that was demolished at 6160 Beach Blvd.

Brooks Rehabilitation Hospital has owned the Beach Boulevard office furniture site since 2000.

"We will be expanding services on our University Boulevard campus and building a skilled nursing facility on the Gator Office site," Jill Matejcek, manager of public and community relations for Brooks Rehabilitation, said in May.

She said construction could start in early 2014.

The skilled nursing facility would sit next to the Brooks Rehabilitation Hospital campus at 3599 University Blvd. S.

Brooks will expand the services provided at its University Boulevard campus to include the skilled-nursing facility serving both patients in need of short-term rehabilitation as well as long-term care, Matejcek said.

Meanwhile, Brooks Rehabilitation has opened its Bartram campus along Bartram Park Boulevard near Old St. Augustine Road and Interstate 95.

Bartram Crossing is a single-story, 100-bed facility fashioned around a "neighborhood concept," comprising two short-stay rehab communities and one long-term community.

The site also features Bartram Lakes, a 61-unit assisted-living facility, and The Green House Residences at Bartram Park, consisting of two 12-bedroom residential homes for seniors with Alzheimer's disease or other dementias.

In May, Matejcek said Brooks served more than 15,000 seniors last year "and knows firsthand that they have unique health care needs."

"Whether it is gaining physical strength and coordination, increasing mental stimulation or receiving emotional and social support, Brooks is committed to furthering their services to transform the lives of seniors in our community," she said.

Cove at St. Johns preparing for sales

The Cove at St. Johns development group applied to the city to put up its temporary sales trailer at 3958 Baymeadows Road at Goodby's Lake.

Prospect Cove Development LLC is the project owner.

Developers of the unfinished condominiums recently filed plans to restart the project, which had been taken over by the Federal Deposit Insurance Corp. in 2009. Prospect Cove Development bought it this summer.

The four buildings are zoned for up to 86 units.

Significant signs

Monument signs were approved for a housing development and a Downtown church.

Brown Enterprises will put up two monument signs for the Basilica of The Immaculate Conception at 121 E. Duval St. Downtown.

And Maier Development Solutions LLC will put one up for Mill Creek at Kendall Town Center in Arlington's Regency area at 1111 Ballard Ridge Road.
