Brooks Rehabilitation Outpatient Clinic – Orange Park is planning an expansion.
The outpatient clinic is at 500 Park Ave. off Wells Road, less than a quarter-mile west of bestbet Orange Park. Brooks Rehabilitation, through Genesis Health Inc., owns the property.
City-owned utility JEA approved a service availability request Nov. 10 for the project, which would add a 7,000-square-foot pediatric feeding and swallowing clinic. The project will also renovate the existing building’s 10,000-square-foot interior to “maximize patient care space,” according to a Brooks spokesperson.
A service availability request indicates interest in a project, but not a done deal. Neither Clay County nor the St. Johns River Water Management District has applications in review.

Brooks said construction is expected to take about a year once building permits are issued. The clinic will remain operational during that time.
A preconstruction meeting with JEA to discuss the expansion is being scheduled.
The medical facility opened in 2015. According to the company’s website, it offers a range of rehabilitative therapy services. Treatments include amputee, orthopedic, neurologic and sports rehabilitation, as well as physical, occupational and speech therapy.
Specialized care is also available for conditions such as lymphedema, breast cancer recovery, concussion, pelvic health and temporomandibular disorders. Additional therapies include cognitive and geriatric rehabilitation, infant feeding support, dry needling, balance and vestibular therapy, and advanced programs such as Vivistim nerve stimulation.

A separate Brooks Rehabilitation campus focused on senior care could also be coming to Middleburg.
Clay County is reviewing site plans for what could become a 307,760-square-foot campus on 43.5 acres at Old Jennings Road and First Coast Expressway. A map of the proposed development was included as part of Brooks’ grading plans.
According to a Sept. 22 statement from Brooks, the master site plan included with its grading application is conceptual, and no building permit applications have been submitted. The project remains under review.
Brooks operates three inpatient hospitals in Florida and more than 70 outpatient locations, including two in Clay County. The organization plans to expand in 2026 with a new facility at Mayo Clinic’s Arizona campus.
