Taylor senior care facilities sold for $21 million

Vivo Healthcare bought the properties that include Taylor Care Center, Taylor Manor Assisted Living and Taylor Villas.

A campus of senior care facilities in south central Jacksonville sold Dec. 11 for $21 million. 

The property at 6535 Chester Ave. includes Taylor Care Center, Taylor Manor Assisted Living, Taylor Villas, Barber Wellness Building and the Samuel C. Taylor Foundation Building.

It is at Powers Avenue and Dupont Station, southwest of University Boulevard and Philips Highway.

The 17.58-acre facility comprises 22 buildings constructed from 1983-86, property records show.

It was sold by the board of trustees of The Samuel C. Taylor Foundation of Jacksonville.

Vivo Healthcare bought the facility through Taylor SNF Propco LLC. It took out a $39.23 million mortgage with Forbright Bank of Chevy Chase, Maryland.

The mortgage said the facility would be renamed Vevo Healthcare Taylor, Vivo Healthcare Taylor Manor and Vivo Healthcare Taylor Villas.

The mortgage said Taylor SNF Propco LLC is based in Lakewood, New Jersey.

Vivo Healthcare manages nursing facilities throughout Florida, according to its website. It has two Jacksonville facilities, Vivo Healthcare University and Vivo Healthcare Normandy, as well as Vivo Healthcare Orange Park. It has other locations in Fort Pierce, Ocoee, Sarasota, Sebring, Wauchula and Winter Haven.

The two Jacksonville locations have a combined 199 beds. 

Taylor Care Center has 120 beds. On average, it serves 74 residents per day, according to medicare.gov.

Medicare.gov rates Taylor Care Center above average, at four out of five stars. Over the last three years, the facility has received no complaints or infection control citations from Medicare.
