The facility is at 150 Village Crossing Court in St. Augustine north of Florida 16 about 4 miles east of Interstate 95. It is east of Woodlawn Road and west of College Drive.

Capital Funding Group of Baltimore sold the property through St. Augustine Real Estate LLC.
The buyer was Quality Senior Housing Foundation Inc. of Atlanta through QSH St. Augustine LLC.
