A sixth ArchWell Health has emerged for Northeast Florida.

Municipal utility JEA is reviewing a service availability determination request for the senior-care clinic at 4750 Soutel Drive in the Harborview area of Northwest Jacksonville. The site is east of New Kings Road and south of Interstate 295.

Fulmer Lucas Engineering of Nashville, Tennessee, is the applicant.

The space is in The Shoppes of Norfolk at southwest Soutel Drive and Norfolk Boulevard.

NAI Hallmark represents the landlord.

A site brochure shows the 52,500-square-foot shopping center is anchored by Family Dollar. ArchWell is not on the plan.

The JEA application, submitted April 21, describes the project as an “existing building with proposed interior build-out for a new health office.”

It says there would be four practitioners and 16 employees.

On March 12, ArchWell Health filed fictitious names with the state Department Division of Corporations to do business in Northeast Florida.

ArchWell Health Medical of Florida LLC of Lithia, a Tampa suburb, is listed in the state filings as the owner. It is part of Nashville-based ArchWell Health Medical Holdings LLC.

Dr. Donald Goddard, M.D., is named on the filings.

ArchWellHealth.com lists Goddard as president and national director of primary care.

ArchWell Health has at least five Northeast Florida locations in the works.

Area locations

The site lists the five area locations as coming soon. ArchWell Health has been building-out the clinics.

The five filings are:

• Archwell Health – Arlington, 1072 Arlington Road N. The Arlington build-out is 8,250 square feet at a project cost of  $2.77 million in a building north of the Arlington Expressway.
