Brooks Rehabilitation campus planned on 43.5 acres in Middleburg

The 300,760-square-foot facility at Old Jennings Road and First Coast Expressway will focus on senior care.

A Brooks Rehabilitation campus focusing on senior care is planned in Middleburg. 

Clay County is reviewing site plans for the 307,760-square-foot campus on 43.5 acres at Old Jennings Road and First Coast Expressway. 

There are 18 buildings planned at the site. They include:

• A 138,000-square-foot, three-story facility offering assisted living and skilled nursing services.

• Four memory care buildings, totaling 32,000 square feet.

• Six medical office buildings, totaling 44,300 square feet.

• Six non-medical office buildings, totaling 44,300 square feet.

• A 48,500-square-foot, two-story hotel designed for visiting families.

Jacksonville-based England-Thims & Miller is the civil engineer. According to the Clay County property appraiser website, Brooks Health System owns the property, purchased in 2015, through Genesis Health Inc. for $5.1 million. 

The undeveloped site, which is about a half mile north of Ascension St. Vincent’s Clay County, requires grading before construction can begin.

Brooks operates three inpatient hospitals in Florida — Brooks Rehabilitation Hospital-University Campus and Brooks Rehabilitation Hospital-Bartram Campus in Jacksonville and Halifax Health | Brooks Rehabilitation Center for Inpatient Rehabilitation in Daytona Beach— and plans to expand in 2026 with a new facility at Mayo Clinic’s Arizona campus. The system also runs more than 70 outpatient locations in Florida, including two in Clay County.

Founded by Dr. J. Brooks Brown, who opened Memorial Hospital in 1969 to address a shortage of beds in Jacksonville, Brooks opened its first independent, freestanding rehabilitation hospital in 1994.

The Clay County Development Review Committee will discuss the project Sept. 25.
