ArchWell Health, a health care service provider for patients 60 and older, is building-out at southwest Soutel Drive and Norfolk Boulevard in Northwest Jacksonville.

ArchWell Health specializes in treating patients with Medicare Advantage coverage.

Hoar Construction LLC is the contractor for the almost $3.55 million project to renovate 14,532 square feet at 4750 Soutel Drive, Unit 10. The city issued a permit Sept. 10.

The site is in the Harborview area, east of New Kings Road and south of Interstate 295.

ArchWell Health Public Relations Senior Analyst Kristine Szarkowitz said Sept. 11 it would open in 2026.

It is one of six identified sites for ArchWell Health in Northeast Florida.

ArchWell Health offers primary care, lab work, vaccinations, care management, preventative health screenings, nutrition education, cardiology telehealth, social work referrals and more.

Five sites have opened since May in Arlington at 1072 Arlington Road N., north of the Arlington Expressway; Jacksonville Heights, 7628 103rd St. in Oak Hill Village in West/Northwest Jacksonville; 5930 Normandy Blvd. in Westside Plaza; 311 Blanding Blvd., Suite 4, in Blanding Plaza in Orange Park; and 5000-4 Norwood Ave. in Gateway Town Center in Brentwood.

Szarkowitz said there no plans for now to open more area clinics.

The Soutel permit specifies that ArchWell Health offers primary care, lab work, vaccinations, care management, preventative health screenings, nutrition education, cardiology telehealth, social work referrals and more.

The plans say the proposed project is a single-story tenant infill of an existing space. The interior build-out of 14,532 square feet in the strip center includes new mechanical, electrical, plumbing and structure reinforcement of the leased space.

There are 10 ArchWell Health centers on Florida’s Gulf Coast. Szarkowitz said five more centers will open in that area in late 2025 and early 2026.

Those 15 along with six in Northeast Florida means ArchWell will operate 21 clinics in the state.

ArchWell operates more than 75 centers in the 12 states of Alabama, Arizona, Arkansas, Colorado, Florida, Kansas, Missouri, North Carolina, Nevada, Nebraska, Ohio and Oklahoma.

If ArchWell is a member’s primary care physician, that member can use any ArchWell location when out of town because all locations have access to medical records.

The website is archwellhealth.com.

ArchWell Health was formed in Nashville, Tennessee, in 2021. It works on a Value Based Care model in contrast to the more traditional fee-for-service model, said Tim Jeans, ArchWell Health regional growth director for Florida, in a previous interview.

In a fee-for-service model, providers bill patients on a per-service or per-procedure basis. Value Based Care services are billed depending on patient outcomes, equity and cost of care, according to the American Medical Association.

ArchWell Health members are encouraged to come in every six or eight weeks, allowing staff to track progress and create more awareness of living a healthy lifestyle.

“We want to identify problems before they cause symptoms or affect the quality of life. So we do a lot of what we call behavioral health screenings,” Jeans said. “So we’re all about picking up early disease when there’s something we can do about it before it impacts people’s lives and encourages creating a healthy lifestyle.”

Members are also invited to drop by unannounced for coffee or to meet with others for games and discussions. This service is aimed at those who have lost a spouse or have few remaining friends.

ArchWell has activity rooms where members can attend nutrition classes.

ArchWell is staffed by board-certified physicians and medical staff. Offices are open 8 a.m. to 5 p.m. Monday through Friday with 24-hour access to doctors on duty via telephone.

It is not affiliated with any area hospitals. If tests or treatments cannot be performed in the office, members will be referred to other medical centers for care.

ArchWell Health says it is a national company “devoted to improving the lives of senior members through excellent primary care.”

It says each primary care center is in a neighborhood “with ease of access and provides an opportunity for seniors to connect with a vibrant, caring community.”

ArchWell Health serves members over the age of 60 and Medicare Advantage patients.

It partners with Medicare Advantage plans that include Aetna, Anthem, Cigna Healthcare, Devoted Health, Humana and United Healthcare.

The ArchWellHealth.com site says members will have appointments averaging 40 minutes; telehealth appointments; on-site lab work; 24/7 access to providers via phone or text message; comprehensive care on-site; access to health records via the online member portal; and a health specialist referral program.
