Ascension St. Vincent’s teams with PathPoint Health

The healthcare providers plan a Southside clinic to treat and manage metabolic conditions.

Ascension St. Vincent’s is joining with PathPoint Health to open a new clinic aimed at treating and managing metabolic conditions such as diabetes and obesity.

The center, at Ascension St. Vincent’s Southside Hospital, 4201 Belfort Road, in the Joe Adams Building, Suite 4030, is expected to open in October, according to a May 1 news release.

“Metabolic conditions like diabetes and obesity affect thousands of people across Northeast Florida, and too often, patients are managing these conditions without coordinated support,” Scott Kashman, interim market lead for Ascension St. Vincent’s, said in the release.

“This partnership with PathPoint Health allows us to offer a more connected approach to metabolic care, right here in our community.”

Services are expected to include medical care, exercise and nutritional planning, lifestyle counseling, diagnostic assessments and care coordination. Both in-person and virtual visits will be offered.

PathPoint Health collaborates with patients’ primary care and specialty providers, it said.  

Ascension and PathPoint say they plan to open additional metabolic clinics in the region.

Partnering with Ascension St. Vincent’s, with its comprehensive footprint across Northeast Florida, enables us to scale this care model quickly and bring it to more communities,” Grant Rutledge, PathPoint founder and CEO, said in the release.

PathPoint Health has three locations in Tennessee, according to its website.
