St. Johns County issued a build-out permit April 16 at a project cost of $660,000 for a 4,260-square-foot Heartland Dental office in St. Augustine,
The permit follows a building permit issued July 17, 2025, at a project cost of $1.2 million.
The dental office is planned at 6070 Florida 16, Unit B, south of SilverLeaf Parkway and about 3 miles northwest of Tocoi Creek High School.
Brahma Construction of Taylorsville, Kentucky is the contractor. The land is owned by First Coast Energy LLP, parent company of the Daily’s convenience store chain.
Heartland Dental, founded in 1997, works with more than 3,000 dentists in more than 1,800 locations in 39 states and the District of Columbia. It is majority-owned by KKR, a global investment firm that owns more than 200 companies with more than $149 billion in investments, according to its website.
Each Heartland office carries the name, logo and brand of the practice it supports. In St. Johns County, they include Smiles at Julington Creek, Durbin Creek Dental Care and Southlake Dental Care, which operates in Saint Johns.

In January, Heartland Dental opened its newest county location, Dental Care at Race Track, about 10 miles north at The Shops at Race Track Rd in Saint Johns.
In Jacksonville, the company began renovating the former Community First Credit Union at 8165 Point Meadows Way along Baymeadows Road near Interstate 295 for an office and clinic. Contractor BHL Services Inc. was issued a build-out permit at a project cost of $873,620 in January.
In Clay County, Heartland Dental began permitting for an office in Middleburg in late December 2024. Duck Creek Dental Care plans an Aug. 19 grand opening at 2524 County Road 220 and is accepting new patients, according to staff.
Orlando-based KPM Franklin is the civil engineer. Comanche Construction LLC of Oklahoma is the contractor.
Dental Care at Race Track and Duck Creek Dental Care are members of the Orahh Care Dental Community, a brand owned by Heartland Dental LLC.
